Please provide our office with your insurance information prior to your appointment so that we can expedite reimbursement. If we are unable to verify insurance coverage you will be asked to pay for your visit at the time of service. Copayments are due on the day of service as well as any amount that has been quoted to you by one of our treatment coordinators.
